• Know your numbers. Track your blood pressure, cholesterol, glucose, and weight and keep them in a healthy range.

It is not enough to be aware of the risk factors associated with heart disease. Take action to prevent it. Visit your healthcare provider to discuss your findings. This online tool is free.
